Tasks
- Participate in interdisciplinary clinic communicating patient
needs to providers and assisting in clinic workflow.
- Obtain patient vital signs, ask screening questions, and assess
patient needs.
- Serve as first line of contact for clinical concerns, providing
patient specific education and triaging patient communications to
the appropriate member of interdisciplinary team.
- Work with patient navigator to ensure scheduling, new patient
intake, and insurance authorizations are completed in timely and

DescriptionEssential Functions:
- Provides nursing care using a Family Centered approach.
- Assesses individualized patient needs and plans, implements,
educates and evaluates care using Evidence-Based Practices.
- Ensures clinic processes and workflows comply with regulatory
and policy compliance.
- Works closely with providers and other team members to ensure a
smooth clinic flow addressing patient/parent concerns and attaining
positive patient and family outcomes.
- Documents according to organizational standards, policies, and
procedures.
- Engages in the "workplace community" by staying aware of
changes, participating in data collection or auditing,
understanding Quality Improvement, and representing staff at
relevant meetings and then reporting important information and
updates back to staff.Education Requirement
- Graduation from an ACEN or CCNE accredited school,
required.
- Bachelor's Degree in Nursing, preferred. Must successfully
complete BSN within 5 years of start date.Licensure
RequirementLicensed as a Registered Nurse in the State of Ohio,

or equivalent combination of education and experience,
