Minimum Requirements Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Current Physical Therapy license in Ohio required to start the assignment. Current CPR certification required. Valid driver’s license, vehicle insurance, and reliable personal vehicle for patient visits. Experience providing home health physical therapy or equivalent outpatient/short-term rehabilitation experience preferred. Ability to complete evaluations and develop plans of care according to state guidelines and CMS documentation requirements. Willingness to participate in weekend rotation and potential availability on observed holidays as required by the agency. Ability to supervise and coordinate with PTAs and home health aides when allowed by state regulation. Assignment Details Facility type: Home Health. Shift name: Other.
